Court form changes - June 2016

HM Court Service has amended a range of Court forms as part of changes to help with fees processes. The following Court forms have been amended to include fields for the new help with fees reference number.

Court forms updated are:-

• N1 – Claim form (CPR Part 7)
• N161 – Appellant's notice (All appeals except small claims track appeals)
• N164 - Appellant's notice (Small Claims Track only)
• N208 - Claim form (CPR Part 8)
• N211 - Claim Form (Additional claims- CPR Part 20)
• N244 - Application notice
• N245 – Application for suspension of a warrant and/or variation of an order
• N316 - Application for order that debtor attend court for questioning
• N323 - Request for Warrant of Control
• N379 - Application for charging order on land (CPR Part 73)
• N461 - Claim form - Judicial Review
