HM Court Service has amended the Adoption form A58 and the supporting notes (A58 Notes).
The new July edition of both form and notes are for use from Monday 24 July 2017.
The existing version (November 2016 edition) of the form will be accepted by HMCTS until 31 July 2017.
The main amendment is to Part 2 of the A58 form which provides greater clarity to applicants about the different types of order a child may be subject to based on the jurisdiction making the order.
Amended forms are:-
Whilst courts will continue to accept previous versions of the form A58 (11.2016) until 31 July 2017, we understand that all applications lodged using the older version on or after 1 August 2017 will not be accepted by HMCTS.
The new July 2017 editions will be available here for use from 24 July.
